Role Summary

The Bilingual Case Manager plays a crucial role in delivering in-person, curriculum-driven home visits to support pregnant women and parents of infants and toddlers. This position emphasizes the enhancement of family capabilities in health improvement, social empowerment, and child development, ultimately guiding families towards school readiness. Responsibilities include offering encouragement, identifying client needs, and facilitating access to essential services.

Core Responsibilities

  • Maintain all necessary credentials and certifications, including Home Visiting CDA credential (if applicable), ServSafe Certification, and First Aid license. Participate in training on maternal and child health, early learning, and related topics as required.
  • Schedule and conduct weekly visits that incorporate activities aimed at enhancing the family's role as their child's primary educator, ensuring a minimum of four visits each month.
  • Employ a culturally-sensitive approach during all client interactions, ensuring respect and professionalism.
  • Deliver educational sessions and resources tailored for pregnant women and families with young children to meet their developmental, health, and educational needs.
  • Connect families with local resources and social services, assisting them in identifying their health and social service needs.
  • Maintain accurate records of all client communications and assessments using designated software.
  • Collaborate with team members to facilitate timely health and educational appointments for clients.
  • Engage in community outreach to promote awareness of EHS services and identify potential clients.
  • Participate in the collection and distribution of essential supplies for EHS clients.
  • Encourage family involvement in EHS events and activities.

Qualifications

  • AA/AAS in Early Childhood Education, Social Services, or a related field, or Infant-Toddler/Home Visiting CDA credential.
  • Bilingual proficiency in Cambodian and English.
  • Valid driver's license and experience in community health.
  • Knowledge of child development, pregnancy, and early childhood education.

Preferred Qualifications

  • BA/BS degree in a related field.
  • Two or more years of experience in community outreach.
  • Experience with maternal and child health issues.
  • Background in fostering positive parent-child interactions.
