Job Summary

As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare providers to develop and implement individualized care plans that promote optimal health outcomes and quality of life.

Key Responsibilities

  • Conduct comprehensive assessments of patients' physical, emotional, and social needs to identify areas for improvement and develop targeted interventions.
  • Collaborate with patients, families, and healthcare providers to establish and achieve care goals, including medication management, wound care, and disease management.
  • Develop and implement individualized care plans that address patients' unique needs and preferences, and provide education and support to patients and families to promote self-management and independence.
  • Monitor patients' progress and adjust care plans as needed to ensure optimal health outcomes and quality of life.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination and transitions.

Requirements

  • Current RN license in the state of practice.
  • 2 years of RN experience, with a focus on home health or hospice care.
  • Basic Life Support (BLS) certification.
  • Valid driver's license and proof of auto insurance.
