Job Summary

The Registered Dietitian serves as a vital member of the PACE Interdisciplinary Team at Element Care. This role works under the direction of the Nutrition Coordinator and the Site Coordinator, providing comprehensive nutrition education and assessments to our PACE participants. As a key contributor to the IDT, the Registered Dietitian participates in the planning, implementation, and evaluation of care plans that meet the objectives, standards, and policies of the PACE model of care.

Key Responsibilities:

  • Assess participants' nutritional status at enrollment, annually, and as needed.
  • Share relevant participant information with the Interdisciplinary Team.
  • Help plan dietary supplements for high-risk participants.
  • Report observations and participant changes to ADH staff clinicians.
  • Participate in family meetings/case conferences as needed.
  • Provide follow-up treatment and teaching as needed.
  • Make home visits as requested.
  • Interact with participants, caregivers, and families in a professional and courteous manner.
  • Interact with other providers and community professionals in a courteous and professional manner.
  • Maintain accurate records and documentation in a timely manner.
  • Ability to pass a fit test; position requires a mask with a tight seal.

Additional Responsibilities:

  • Participate in Quality Improvement projects and assessments as needed.
  • Participate in Element Care Committees as required and communicate relevant information as necessary.
  • Complete required annual Health Stream trainings in a timely manner.
  • Adhere to all policies and procedures and support the Primary Vision Statement.
  • Adhere to the confidentiality policy of Element Care, including HIPAA guidelines for participant information.
  • Be flexible to perform other related tasks as reasonable.

Essential Job Requirements:

  • Master of Science Degree in Nutrition (preferred)
  • Registered Dietitian through CDR and current state licensure in the Commonwealth of Massachusetts.
  • Two years of experience working in a community setting.
  • One year of experience working with the geriatric population.

  • Ability to lead independently and work well with others.
  • Ability to work within a team concept and be considerate of co-workers and colleagues.

Preferred Skills:

  • Ability to pay attention to detail and be a self-starter.
  • Ability to quickly recognize situations/problems before they escalate.
