As a Phone Triage Registered Nurse in the Heart Center, you will play a crucial role in assessing patients' needs over the phone, ensuring timely identification of individuals with potentially serious health conditions. You will act as a vital link between patients, physicians, and healthcare facilities.
Key Responsibilities:
- Engage with patients via telephone to gather essential information and direct them to appropriate care settings, including emergency rooms or urgent care facilities.
- Conduct preliminary evaluations to determine the urgency of the patient's condition, including understanding their primary concerns and, when possible, collecting vital signs and mental status information.
- Collaborate with physicians to relay critical patient information.
- Promptly refer patients with high-risk symptoms, such as chest pain, to emergency services as directed by the physician.
- Coordinate appointments for patients who require follow-up care but do not need immediate emergency intervention.
- Manage prescription renewals as per physician directives.
- Serve as a liaison for referring physicians seeking to discuss mutual patients with cardiologists.
- Facilitate communication between hospitals and cardiologists, ensuring timely consults and patient transfers.
- Participate actively in the Emergency Response Team.
- Administer IV Lasix as needed.
- Exemplify and uphold the core values of Respect, Excellence, Accountability, Compassion, and Honor.
- Maintain consistent and reliable attendance.
- Perform additional duties as required.
Qualifications:
- Associate's Degree in Nursing; Bachelor's Degree preferred.
- 1-2 years of experience in Nursing Triage or equivalent roles, with a preference for backgrounds in Cardiology, ICU, ED, or OR.
Knowledge, Skills, and Abilities:
- Proficient in telephone-based clinical assessment techniques.
- Familiarity with medical practice telephone triage protocols.
- Understanding of home health information for patients awaiting physician visits.
- Competence in utilizing electronic medical records for patient history checks.
- Adept at using computerized medical databases for informed decision-making.
- Ability to make quick and effective triage decisions in emergency situations.
- Skilled in IV administration.
Licensure and Certifications:
- Valid New York State RN License.
- Current ACLS Certification.
- Current BLS Certification.
