As a Home Health LPN, you will play a vital role in delivering compassionate and personalized care to our patients, working collaboratively with our interdisciplinary team to achieve positive patient outcomes.
Responsibilities- Provide direct patient care under the supervision of a registered nurse, following the plan of care and implementing current nursing practices.
- Implement treatments and work collaboratively with the healthcare team to achieve positive patient outcomes.
- Document patient services accurately and timely, reflecting the plan of care.
- Evaluate patient needs and provide education and information pertinent to diagnosis and individualized plan of care.
- Use and prepare equipment and supplies effectively and efficiently.
- Provide pain and symptom management, evaluating patient response to treatments and medications.
- Participate in personal and professional growth and development, as well as organization-wide quality assessment and performance improvement teams and activities.
- Attend required staff meetings and patient care coordination activities.
- Perform other duties as assigned by the Registered Nurse or Clinical Manager.
- Graduate of an accredited practical nurse or vocational nursing program.
- One to two years of nursing experience, preferably in community health or medical/surgical settings.
- Current licensure as an LPN in the state of practice.
- Competitive hourly rate of $38-$42, plus mileage reimbursement and benefits.
