Key Responsibilities
  • Coordinate Patient Care: Develop and implement individualized care plans, ensuring seamless transitions and effective communication with patients, families, and healthcare providers.
  • Assess and Evaluate: Conduct thorough assessments, identify patient needs, and develop strategies to address complex care requirements.
  • Lead and Manage: Supervise and direct field teams, ensuring the delivery of safe, effective, and compassionate hospice services.
  • Collaborate and Communicate: Foster strong relationships with patients, families, and healthcare professionals, promoting open communication and coordination of care.
Requirements
  • Education and Licensure: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing, with a current State License as a Registered Nurse.
  • Experience: 2-3 years of experience as a registered nurse in a clinical care setting, with 1 year of supervisory experience.
  • Skills and Qualities: Excellent communication and interpersonal skills, ability to manage patient care episodes, and a strong commitment to delivering exceptional patient care.
Preferred Qualifications
  • BSN: Bachelor of Science in Nursing degree.
  • Hospice Experience: Experience working in a hospice or home health environment.
  • Technical Skills: Familiarity with Home Care Home Base (HCHB) software.
