Job Opportunity

We are seeking a compassionate and dynamic Licensed Practical Nurse to join our Hospice team at AdvisaCare. As a Hospice LPN, you will play a vital role in providing emotional support and care to patients and their families.

Key Responsibilities:

  • Conduct initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs.
  • Implement individualized plans of care and make revisions as needed.
  • Ensure compliance with AdvisaCare Hospice policies and procedures, as well as Medicare requirements.
  • Consult with and educate patients and families on disease processes, self-care techniques, end-of-life care, and ethical concerns.
  • Supervise paraprofessionals providing services to patients.
  • Prepare clinical and progress notes, participate in interdisciplinary group conferences, and maintain high levels of customer satisfaction.

Requirements:

  • Current Arizona license in good standing.
  • Minimum 1 year of hospice experience.
  • Empathetic approach to patients and caregivers.
  • Ability to adapt to patients' status and needs on a full-time on-call basis.
