Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team at the Veterans Affairs, Veterans Health Administration. As a Homeless Outreach Social Worker, you will play a critical role in identifying and engaging with Veterans in need, linking them to essential services, and promoting psychosocial stabilization and increased residential stability.

Key Responsibilities
  • Conduct thorough assessments of Veterans' needs and develop effective treatment plans to address their unique challenges.
  • Collaborate with community agencies and other government programs to provide comprehensive support services to Veterans.
  • Facilitate referrals and coordinate services to ensure seamless care and support for Veterans.
  • Provide crisis intervention and conflict resolution skills to effectively manage challenging situations.
  • Maintain accurate and up-to-date records of Veteran interactions and services provided.
Requirements
  • Master's degree in Social Work from a fully accredited school of social work.
  • Licensure or certification as a social worker in a state at the independent practice level.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Strong knowledge of community resources and ability to make appropriate referrals.
  • Ability to work independently and as part of a team, with a high level of professionalism and discretion.
