Job Summary:
Avail Senior Living is seeking a skilled Licensed Practical Nurse to join our team. As a LPN, you will be responsible for providing high-quality resident care and services.
Key Responsibilities:
Provide direct resident services and medication management as needed.
Assist in the implementation of and/or provide additional information for the resident service plan and update them on an ongoing basis.
Oversee the provision of assistance with ADLs to residents whose service plan indicates the need.
Communicate pertinent resident information to co-workers when leaving the community and/or at shift changes.
Assume responsibility for quality resident care and seek guidance from supervisory staff when necessary.
Communicate with physicians/other health providers regarding the resident's health status when appropriate.
Determine when a resident requires room service for meals due to illness.
Maintain accurate and complete resident file documentation.
Schedule tests ordered by the physician, assist residents in scheduling medical appointments and transportation.
Visit, residents who are ill and provide/arrange necessary follow up.
Respond immediately to emergency situations or resident calls.
Welcome and orient new residents to the building and services available in the community.
Provide educational support and guidance to residents and their families.
Assist with monthly Wellness checks.
Perform other duties as directed.
Assist with ADL assessments.
Take calls as directed by wellness director.
Requirements:
High school graduate or equivalent.
Must have a valid, unrestricted nursing license in the state.
Must demonstrate competency in the following areas: staff supervision, resident care skills, medication management principles and practices, written and verbal communication skills, current CPR certification, and maintain CEU requirements.
Must have the strength and endurance required to sit, bend, lift, and move intermittently during working hours.
Works beyond normal hours, weekends, evenings, holidays, and severe weather situations when necessary.
Is subject to potential exposure to infectious waste, disease, conditions, etc., including TB, HIV, and Hepatitis B.
