Job Summary

We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at Danbury Health Systems. As a key member of our Behavioral Health unit, you will play a vital role in providing exceptional care to our patients and their families.

Responsibilities
  • Provide Patient Care: Under the direction of a Registered Professional Nurse, you will provide patient care that meets the highest standards of quality and safety.
  • Lead Groups and Activities: You will lead and co-lead groups, ensuring a safe and therapeutic environment for patients to engage in activities and express themselves.
  • Communicate Effectively: You will communicate with patients, visitors, and colleagues in a courteous and respectful manner, taking into account individual needs and preferences.
  • Document Patient Information: You will accurately document patient data, including vital signs, behaviors, and any changes in condition.
  • Maintain a Safe Environment: You will ensure a safe and secure environment for patients, staff, and visitors, adhering to Norwalk Hospital Policies and Procedures.
  • Perform Other Duties: You will perform other duties as assigned, including assisting with unit needs and fulfilling compliance responsibilities.
Requirements
  • Education: Associate's degree in Psychology, Human Services, or a related field of study, or CNA with experience working with a psychiatric population.
  • Experience: One year of related experience, with a strong understanding of patient care principles and practices.
  • Skills: Ability to multi-task, prioritize, and adapt to an ever-changing environment, with excellent communication and interpersonal skills.
