We are seeking a highly skilled and compassionate Licensed Vocational Nurse (LVN) to join our team at the Center for Compassionate Care in Hospice El Paso, Inc. As an LVN, you will play a vital role in providing high-quality, patient-centered care to individuals with terminal illnesses and their families.

Key Responsibilities
  • Provide skilled nursing services to patients in our Center for Compassionate Care, including assessing, planning, implementing, and evaluating individual care plans.
  • Work collaboratively with our interdisciplinary team to ensure seamless care coordination and communication.
  • Assess and document current pain levels and intervention outcomes with every contact and PRN.
  • Develop and implement individualized care plans that address the physical, psycho-social, and spiritual needs of patients.
  • Provide education and support to patients and their families to promote optimal health outcomes.
  • Collaborate with our medical team to ensure accurate and timely documentation of patient information.
  • Participate in weekly patient care conferences to review patient progress and adjust care plans as needed.
Requirements
  • Current LVN licensure in the state of Texas.
  • Minimum 1 year of experience in hospice care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively as part of a team.
  • Strong problem-solving and critical thinking skills.
  • Ability to maintain confidentiality and handle sensitive patient information.
