Job Summary

As a Social Worker with LifeBridge Health, you will play a vital role in ensuring that patients and their families receive comprehensive, family-oriented services in the comfort of their own homes. Your expertise will be essential in assessing patients' and families' psychosocial, environmental, and financial needs, and developing a plan of care that addresses their unique needs.

Responsibilities
  • Assess patients' and families' needs and develop a plan of care that addresses their unique needs
  • Provide counseling services to patients and their families to help them cope with terminal illness
  • Coordinate with community and agency homecare referrals for services
  • Collaborate with other caregivers to ensure comprehensive care
Requirements
  • Master's Degree in Social Work from an accredited school
  • Minimum of three years of clinical experience, with one year in a community health setting preferred
  • LCSW or LCPC certification
