About the Job

We are seeking an experienced and compassionate professional to join our team as an FCCP Transition Assistance Coordinator. As a member of our staff, you will play a key role in supporting families through the transition process, helping them to access resources and services that promote their well-being and success.

 

Key Responsibilities

  1. Develop and implement individualized service plans that meet the unique needs and goals of each family.
  2. Collaborate with families, Social Workers, and other stakeholders to ensure seamless transitions and successful outcomes.
  3. Provide ongoing support and advocacy to families, empowering them to make informed decisions and take control of their lives.
  4. Coordinate services and resources with community-based service providers to promote family stability and well-being.
  5. Maintain accurate records and reports to track progress and evaluate program effectiveness.

 

Requirements and Qualifications

  • Bachelor's degree in social work or a related field.
  • Experience in child welfare, parent education, home-based services, and/or crisis intervention.
  • Knowledge of child and adolescent development, family dynamics, substance abuse, home management, and parenting skills.
  • Bilingual in Spanish preferred.
  • Strong organizational, critical thinking, and time-management skills.
  • Basic computer skills and knowledge of Microsoft Office 365.
  • Ability to communicate effectively with diverse populations and work collaboratively as part of a team.

 

Salary and Benefits

The estimated salary for this position is $55,000 - $70,000 per year, depending on experience and qualifications. We offer a comprehensive benefits package, including health insurance, retirement plan, and paid time off.
