We are seeking an experienced Case Manager to join our team in Middletown, RI. As a Case Manager, you will be responsible for actively engaging with clients to reduce barriers to achieving positive outcomes in critical areas of life. This includes participating in daily team meetings, attending client meetings, and providing support with understanding medications and finding solutions to challenges such as transportation to appointments or communication with Social Security for benefits.

The ideal candidate will have a minimum of 1-2 years of experience in the Human Services field, an Associate Degree in a related field, and exceptional verbal and writing skills. You will also be self-motivated and independent, with the ability to prioritize work to meet deadlines.

The estimated salary for this position is $60,000 - $70,000 per year, depending on experience. We offer a comprehensive benefits package, including excellent health and dental insurance, vacation time, and retirement program through Mutual of America.
