Hospice LPN Job Description

The Hospice LPN/LVN is a vital member of our healthcare team, responsible for delivering high-quality care and support to terminally ill patients and their families. This role requires a compassionate and skilled nurse who can provide comfort, dignity, and respect to those in need.

Key Responsibilities:
  • Develop and implement individualized care plans for patients, taking into account their unique needs and goals.
  • Provide hands-on care, including administering medications, wound care, and other treatments as needed.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Document patient progress, including vital signs, medication administration, and other relevant information.
  • Participate in patient and family education, providing guidance and support to help them navigate the hospice care process.
  • Contribute to the development and implementation of policies and procedures to ensure high-quality care and patient safety.
Requirements:
  • Current LPN/LVN licensure in the state of practice.
  • Compliance with all relevant professional standards of practice.
  • Participation in Amergis' Competency program, as applicable.
  • Current CPR certification, if applicable.
  • TB questionnaire, PPD, or chest x-ray, if applicable.
  • Current Health certificate, as per contract or state regulation.
  • Meet all federal, state, and local requirements.
  • Successful completion of new hire training, as applicable to the job site.
  • Understanding of patient confidentiality and HIPAA requirements.
  • Ability to effectively communicate with patients, families, and healthcare team members.
