Job Title: Regional LPN

We are seeking a skilled and dedicated Regional LPN to join our team at Diversicare Healthcare Services & Diversicare Ther. As a Regional LPN, you will play a vital role in providing exceptional care to our residents and supporting our centers in the region.

Job Summary:

The Regional LPN will be responsible for providing skilled technical nursing services to residents within an assigned unit, supporting medical care staff, and supervising CNAs to ensure a high quality of care is provided. This role requires strong leadership and communication skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Provide skilled technical nursing services to residents within an assigned unit
  • Support medical care staff and supervise CNAs to ensure high-quality care
  • Lead and motivate subordinates to achieve positive outcomes for residents
  • Implement and maintain policies and procedures to ensure safe work practices and infection control techniques
  • Participate in the Continuous Quality Improvement Program and ensure follow-up occurs
  • Report accurately to oncoming shift Charge LPN's regarding changes in resident status and follow-up needs
  • Provide input for CNA performance evaluations and coach, counsel, and discipline as needed
  • Recommend staffing, equipment, and supply needs to DNS
  • Capture charges for supplies issued to residents on that unit
  • Participate in care planning process and ensure the nursing component is implemented
  • Ensure the rights of residents are respected and coordinate with other departments as needed
Requirements:
  • Current, unrestricted license by the State Board of Nursing, as an LPN/LVN
  • Ability to lead and motivate subordinates
  • Ability to perform a variety of duties characterized by frequent change
  • Able to work intimately with residents, physicians, and staff
  • Capable of working to prescribe nursing standards
  • Good interpersonal and communication skills
  • Valid driver's license and current auto insurance required
