Job Title: Team Lead/Discharge Coordinator

Counseling & Recovery Services of Oklahoma is seeking a highly skilled and compassionate Team Lead/Discharge Coordinator to join our team at our YES Tulsa location.

About the Role:

This is a challenging and rewarding opportunity to make a meaningful difference in the lives of patients and their families. As a Team Lead/Discharge Coordinator, you will be responsible for coordinating the discharge process for clients, ensuring a smooth transition to the next phase of treatment. You will work closely with the treatment team to develop comprehensive discharge plans, make referrals as needed, and provide ongoing support to clients and their families.

Responsibilities:
  • Conduct initial phone contact with clients and consult with the treatment team to determine the best course of action.
  • Develop and implement comprehensive discharge plans, including referrals to community resources and follow-up appointments.
  • Coordinate with the treatment team to ensure a smooth transition of clients to the next phase of treatment.
  • Provide ongoing support to clients and their families, including answering questions and addressing concerns.
  • Collaborate with the Program Director and other staff members to ensure the effective delivery of services.
Requirements:
  • Bachelor's degree in social or human sciences from a recognized university.
  • Minimum of 5 years of experience in the mental health field or related social service field.
  • Certified Case Manager II or ability to obtain upon hired preferred.
  • Ability to work irregular hours as needed to ensure quality standards are met in service delivery.
  • Current Oklahoma Driver's License and good driving record.

Special Knowledge, Skills, and Abilities:
  • Ability to establish working relationships with clinical, administrative, and support staff as well as community partners.
  • Good working knowledge of community social service resources, basic crisis management skills, and behavioral health resources.
  • Ability to provide trauma-informed care and a commitment to the Sanctuary Model components.
  • Ability to communicate effectively with staff, clients, and the community.
Physical Requirements:

This position requires the ability to stand, walk, sit for extended periods, and lift up to 25 pounds. The employee may be required to travel locally and out of town as needed.
