Licensed Clinical Social Worker

We are seeking a highly skilled Licensed Clinical Social Worker to join our multidisciplinary team at First Choice Community Health. As a key member of our team, you will provide direct therapeutic services to patients, including individual, couple, family, and group therapy sessions. Your expertise will be essential in assessing, treating, and discharging patients, while also advocating for their needs and connecting them with community resources.
Key Responsibilities- Provide individual, couple, family, and group therapeutic services to patients, including assessments, treatment plans, and discharge planning.
- Advocate for patients' needs and connect them with community resources, such as mental health services, substance abuse treatment, and social services.
- Collaborate with multidisciplinary team members, including physicians, medical professionals, and therapists, to ensure comprehensive patient care.
- Maintain accurate and timely documentation of patient interactions, including biopsychosocial assessments and treatment planning.
- Develop and implement evidence-based practices, including trauma-informed care, to address the unique needs of co-occurring populations, particularly children and adolescents.
- Provide emotional and mental health counseling to groups or individuals, building strong rapport and making accurate assessments of their problems.
- Keep detailed and concise records of each patient during the course of their treatment, ensuring confidentiality and adherence to HIPAA guidelines.
- Licensed Clinical Social Worker (LCSW) from the North Carolina Board of Behavioral Sciences.
- Master's Degree in Social Work or related field from an accredited university.
- Minimum two years post-licensure experience in a clinical setting.
- Knowledge and experience with evidence-based practices, including trauma-informed care framework.
- Ability to work effectively with people of diverse cultures, ages, and economic backgrounds in a culturally competent and sensitive manner.
- Strong clerical/administrative skills, including personal computer skills (Microsoft Office software).
