Job Summary

The Case Manager Registered Nurse is responsible for ensuring high-quality, patient-centered care by managing rehabilitative care to ensure optimum outcomes. This role involves coordinating efforts within the rehabilitation team and collaborating with the rehabilitation program director and facility case management director.

Key Responsibilities
  • Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
  • Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
  • Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
  • Document the findings of the discharge planning evaluation and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
  • Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
Requirements
  • Minimum of 3 years in nursing practice, with clinical knowledge, IT skills, effective interpersonal communication, critical thinking, successful problem-solving, ability to work autonomously, innovative thinking, and resourceful navigation of community resources.
  • Preferred experience in nursing practice, with 3 years of care management, community or ambulatory care management, primary care clinical experience, and inpatient discharge planning work.
  • Required education: Bachelor of Science in Nursing or bachelor's degree in a related field. If RN, effective 3/1/18, Bachelor of Science in Nursing or enroll in a BSN or advanced degree program within 6 months and obtain a BSN within 4 years of hire date into RN position.
  • Preferred education: Master's in Nursing.
  • Required license: If RN, must have and maintain current licensure as a registered nurse with the North Carolina Board of Nursing.
  • Required certification: BLS Healthcare Provider.
  • Preferred certification: Level I, nationally recognized certification in Case Management, such as ACM, CCM, or ANA certification.
