Job Summary

Cottesmore of Life Care is seeking a highly skilled and experienced RN Care Manager to join our team. As a key member of our care management team, you will be responsible for coordinating patient care and services, promoting quality care, and ensuring a safe transition from our facility to the next provider or care setting.

Key Responsibilities
  • Coordinate patient care and services, including case management, utilization review, and discharge planning.
  • Work collaboratively with patients, families, care teams, payers, and external entities to promote quality care and ensure a safe transition.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed.
  • Communicate effectively with patients, families, and care teams to ensure seamless care coordination.
  • Collaborate with interdisciplinary teams to promote quality care and improve patient outcomes.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements to ensure compliance and excellence in care management.
Requirements
  • Nursing diploma (associate's or bachelor's degree in nursing)
  • Currently licensed/registered in applicable State
  • One (1) year of clinical experience in post acute care setting preferred
  • Prior case management, utilization review, and discharge planning experience preferred
  • Certified Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) preferred
Preferred Qualifications
  • Expert knowledge in field of practice
  • Excellent organizational skills and ability to prioritize and manage time and assignments
  • Ability to generate written communication that is clear, concise, and well-organized
  • Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility
