Job Summary

We are seeking a highly skilled and experienced RN Case Manager to join our team at Norwalk Hospital Association. As a Clinical Care Coordinator, you will play a critical role in ensuring the efficient progression of care for patients in the Emergency Department.

Key Responsibilities
  • Manage the Utilization Review process for patients requiring hospitalization, ensuring the correct level of care and optimal initial care plan.
  • Make recommendations for Bed Request orders based on thorough review of clinical information and evidence-based screening criteria.
  • Collaborate with Social Work Care Coordinators to develop optimal plans of care for discharge.
  • Analyze clinical, financial, and social situations to identify problems and establish alternative courses of action.
  • Perform rounding with ED team members to ensure appropriate case identification and planning discussions.
  • Advise and educate patients and families on hospitalization guidelines and discharge planning options.
  • Issue required regulatory notices, such as Condition 44 and HINN, as necessary.
  • Offer suggestions for improving performance and operations.
  • Guide and direct the healthcare team to ensure optimal outcomes.
  • Monitor ED throughput and assist in facilitating testing and procedures.
Requirements
  • Registered Nurse license in the State of CT with a minimum of one year of acute care experience.
  • Associate's Degree in Nursing.
  • Knowledge and expertise in electronic medical records and experience with Interqual and Milliman screening criteria.
  • Excellent collaboration and communication skills.
Preferred Qualifications
  • Certification in Case Management.
  • Bachelor's Degree in Nursing.
Work Environment

This position requires a medium to heavy effort, with the ability to exert up to 35 pounds of force. The work environment is generally pleasant, with some manual skills and motor coordination required.
