Position Overview:
The Ambulatory Care Pharmacist will work closely with healthcare providers and support staff to enhance the health outcomes of patients within the organization.
Key Responsibilities:
- Collaborate with primary care teams to optimize medication management.
- Provide expert drug therapy management services, assisting both medical personnel and patients with drug-related inquiries.
- Develop and maintain strong relationships with patients, ensuring their needs are prioritized.
- Assess and address potential medication therapy issues identified through various sources.
- Conduct patient interviews to gather essential information regarding medication usage, allergies, and sensitivities, documenting findings in the patient's medical records.
- Consult with healthcare professionals regarding patient care, including clinical diagnoses, medication combinations, and dosage considerations.
- Guide pharmacy interns, residents, fellows, pharmacy technicians, and other pharmacy staff.
Qualifications:
- Doctor of Pharmacy (PharmD) from an accredited pharmacy college or equivalent education/experience.
- Valid Pharmacist License in the state of Washington upon hire.
- Minimum of two years of experience in Ambulatory Pharmacy.
Preferred Qualifications:
- Completion of a PGY1 or PGY2 residency or equivalent education/experience.
- Certification as a Diabetes Educator or Hypertension Clinician (or willingness to obtain).
