Job Title: Full Time Registered Nurse- Home Health

Job Summary:

Aveanna Healthcare is seeking a skilled and compassionate Registered Nurse to join our team as a Full Time Registered Nurse- Home Health. As a Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with healthcare teams to develop and implement personalized care plans.

Responsibilities:

  • Develop and implement individualized care plans in accordance with physician orders and client needs.
  • Provide skilled nursing care, including wound care, infusions, catheter care, and PICC line dressing changes.
  • Conduct thorough assessments and evaluations to identify client needs and develop effective care plans.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and training to clients and their families on disease management and self-care techniques.

Requirements:

  • Active RN license in the state of application.
  • Valid CPR certification.
  • Valid driver's license.
  • Medicare Skilled Nursing experience preferred.
  • Basic understanding of Oasis.
  • 1-year RN experience in a healthcare setting.
