Job Summary:The Registered Nurse - Home Health is responsible for delivering skilled nursing care to patients in their homes, adhering to established care plans and physician orders. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Key Responsibilities:- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Provide education and training to patients and caregivers on disease management, medication administration, and other health-related topics.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Coordinate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Document patient progress and care provided in accordance with regulatory requirements and company policies.
- Active RN license in the state of application.
- Valid CPR certification and driver's license.
- Medicare Skilled Nursing experience preferred.
- Basic understanding of Oasis and electronic medical records.
- 1-year RN experience in a healthcare setting.
