We are seeking a skilled and compassionate Weekend LVN/LPN to join our Home Health team at AccentCare, Inc.

Key Responsibilities:

  • Provide high-quality nursing visits to patients in their homes, following treatment plans prescribed by physicians.
  • Maintain accurate and complete medical records, ensuring compliance with regulatory requirements.
  • Collaborate with healthcare teams to coordinate patient care and achieve optimal outcomes.
  • Conduct thorough assessments, develop care plans, and implement interventions to address patient needs.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.

Requirements:

  • One year of clinical experience and one year of home health experience.
  • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of operation.
  • Valid CPR certification.
  • Ability to travel to all business locations.

What We Offer:

  • Competitive hourly rate: $30.00-$37.00.
  • Full-time schedule: Wednesday-Sunday, 4-10 hour shifts.
