Job Description

We are seeking an experienced Ambulatory Care Nurse to join our team at WakeMed Health & Hospitals. As a key member of our healthcare team, you will provide exceptional care management services to our patients and their families.

Key Responsibilities
  • Develop and implement individualized care plans for high-risk patients
  • Foster strong relationships with patients, families, and healthcare providers
  • Participate in interdisciplinary team meetings to coordinate care
Requirements and Qualifications
  • Current RN licensure in North Carolina
  • Degree in nursing (diploma, associate's, or bachelor's)
  • At least 5 years of nursing experience
Compensation and Benefits

An attractive compensation package, including a salary range of $85,000 - $115,000 per year, plus benefits and opportunities for career advancement.
