Practical Nurse Team Leader

Position Overview:
We are seeking a dedicated and enthusiastic Licensed Practical Nurse (LPN) to take on the role of Careteam Supervisor. Join our supportive team where we prioritize a positive work environment and maintain an excellent resident-to-staff ratio, offering 12-hour shifts.
Key Responsibilities:
This role involves delivering health services and care to residents within the community. You will actively participate in the development and execution of personalized service plans for each resident while serving as a mentor and guide for nursing assistants and medication aides.
General Nursing Duties:
- Provide comprehensive care and treatment for residents in accordance with your licensure and state regulations.
- Conduct nursing evaluations and assessments for current and prospective residents as mandated by federal and state guidelines.
- Ensure that infection control protocols are understood and adhered to by all staff members.
- Administer medications and treatments, monitor outcomes, and document findings.
- Act as a liaison and advocate for residents, collaborating with physicians and external nursing personnel.
Team Leadership:
- Complete shift reports with team members to ensure effective communication regarding resident care needs.
- Supervise team activities by conducting rounds and providing coaching on service delivery and process enhancements.
Customer Engagement:
- Serve as a primary contact for resident health care services, proactively communicating any changes in resident conditions to all relevant parties.
- Engage with families to discuss the health care services available within the community.
- Promote the range of services offered to external medical providers.
Minimum Qualifications:
- Valid Licensed Practical Nurse or Licensed Vocational Nurse certification.
- Current CPR and First Aid certification.
- Physical ability to lift up to 50 pounds occasionally.
- Willingness to work in varying environmental conditions.
