Per Diem Speech Language Pathologist

At CenterWell Home Health, we are seeking a highly skilled Per Diem Speech Language Pathologist to join our team of dedicated healthcare professionals.

Key Responsibilities
  • Evaluate and provide speech/language pathology services to patients in the home or facility setting, ensuring comprehensive care and optimal patient outcomes.
  • Collaborate with healthcare teams to develop and review treatment plans, promoting seamless care coordination and effective communication.
  • Utilize professional expertise to assess and treat disorders of speech, voice, language, hearing, and swallowing, applying evidence-based practices and industry best standards.
  • Administer and interpret diagnostic tests, and apply therapeutic treatments to address patient needs, ensuring accurate and timely documentation.
  • Monitor patient progress, reporting changes and responses to treatment to supervisors and physicians, fostering a culture of collaboration and care.
  • Provide education and training to patients on alternative communication systems, empowering them to communicate effectively and independently.
  • Offer guidance and support to patients, families, and healthcare staff, promoting a culture of empathy and understanding.
  • Participate in ongoing professional development, staying current with industry best practices and regulatory requirements, ensuring compliance and excellence in care delivery.
Requirements
  • Minimum of six months experience as a speech therapist/speech language pathologist, with a strong background in home health or related settings.
  • Current and unrestricted license to practice, with CPR certification and valid driver's license.
  • Excellent organizational and communication skills, with the ability to work effectively in a fast-paced environment.
