Job Summary

We are seeking a highly motivated and compassionate Sponsored Residential Direct Support Professional to join our team at Heart Havens, Inc. As a key member of our care team, you will provide essential support to individuals with developmental disabilities in their home, promoting their independence, dignity, and overall well-being.

Key Responsibilities
  • Provide personalized support to individuals in their home, ensuring their needs are met and their rights are respected.
  • Develop and maintain Person-Centered Individualized Support Plans (PT V) and other relevant support plans to ensure individuals receive tailored support.
  • Ensure compliance with regulatory requirements, including the Office of Licensing and Human Rights, DMAS, and other relevant agencies.
  • Collaborate with colleagues, families, and healthcare professionals to ensure seamless support and care.
  • Document all interactions, incidents, and support provided in a timely and accurate manner.
  • Participate in routine emergency drills and respond to emergency situations according to policy and procedure.
Requirements
  • High school diploma or GED with at least one year of previous direct support experience in a DD residential setting preferred.
  • Ability to understand and follow service requirements, manage time, and interact with individuals with varying characteristics, diagnoses, and abilities.
  • Knowledge of the DD Waiver, HCBS setting rules, Human Rights, and Office of Licensing regulations.
  • Ability to perform CPR/First-Aid, behavior management interventions, and medication management.
  • Must be able to follow policies and procedures, maintain confidentiality, and demonstrate Heart Havens' mission and values.
