Job Summary

The Licensed Practical Nurse II will provide direct and individualized nursing care to an assigned group of patients under the direction of a registered nurse or provider. The LPN II will observe, report, and record medical information and nursing care provided in the electronic medical record. The LPN II will instruct patients and families according to the teaching plans. The LPN II will communicate with the healthcare team to facilitate the coordination of individualized patient care.

Responsibilities
  • Provide direct and individualized nursing care to patients under the direction of a registered nurse or provider.
  • Observe, report, and record medical information and nursing care provided in the electronic medical record.
  • Instruct patients and families according to the teaching plans.
  • Communicate with the healthcare team to facilitate the coordination of individualized patient care.
Requirements
  • Currently licensed to practice as a licensed practical nurse by the state of employment or holds a multistate LPN license through the interstate Nurse Licensure Compact or eligible for licensure in the state of employment.
  • One (1) year of LPN experience.
  • Basic Life Support (BLS) required prior to completion of orientation.
  • Act 31 training completion required within 45 days of hire.
