Job Summary

We are seeking a highly motivated and compassionate Housing Coordinator to join our team at the Georgia Department of Public Health. As a key member of our team, you will play a vital role in supporting individuals and families in achieving stable housing and improving their overall well-being.

Key Responsibilities
  • Act as a client advocate to coordinate required services and resolve crises
  • Counsel clients in individual and group sessions to help them overcome dependencies and adjust to life
  • Identify environmental impediments to client progress through interviews and review of patient records
  • Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plans
  • Organize support groups or counsel family members to assist them in understanding and dealing with the client
  • Perform clinical assessments of the behavioral health needs of consumers presenting for services
  • Provide case management services related to identified treatment needs
  • Provide training to direct care staff in implementing behavior modification techniques and treatment plans
  • Refer clients or family to community resources to assist in recovery from mental or physical illness and provide access to services such as financial assistance, legal aid, housing, job placement, or education
Requirements
  • Master's degree in a related field from an accredited college or university and eligibility to be licensed
  • Or bachelor's degree in a related field from an accredited college or university and two years of experience in social service delivery
  • Or five or more years' experience with a high school diploma and/or associate degree
Preferred Qualifications
  • Excellent verbal and written communication skills
  • Excellent computer skills, experience with electronic medical records a plus
  • Experience with CAREWare/HMIS a plus
  • Ability to maintain HIPAA compliance, corporate compliance, and client confidentiality
