Job Summary:

This role involves working collaboratively with the healthcare team to ensure the safe and effective use of medications. The ideal candidate will have strong communication skills and be able to provide medication education to patients and staff.

Responsibilities:

  • Review and verify medication orders for accuracy and appropriateness.
  • Prepare and dispense medications in accordance with hospital policies and procedures.
  • Collaborate with physicians and nurses to optimize medication therapy and patient outcomes.
  • Provide medication counseling to patients and their families.
  • Monitor and manage drug therapy for patients with complex medical conditions.
  • Participate in medication safety initiatives and quality improvement projects.
  • Stay up-to-date with the latest pharmaceutical developments and treatment guidelines.
  • Maintain accurate and complete patient records.
  • Provide discharge prescriptions and employee prescriptions and counseling.
  • Responsible for various 340B optimization tasks.
