Job Summary
We are seeking a dedicated and compassionate Clinical Social Worker to join our team in the Infectious Diseases Clinic. As a vital member of the healthcare team, you will be responsible for providing comprehensive substance use assessment and treatment, medical case management, mental health, and referral services to adults living with HIV infection.

Key Duties
- Conduct assessments, develop patient-centered care plans, facilitate resources, monitor progress, and evaluation.
- Provide crisis intervention and emotional support, and create effective plans for resolution.
- Monitor and evaluate patient progress, measure outcomes, and make necessary adjustments to care plans.
- Advocate for patients' needs within the healthcare system and the community, ensuring access to appropriate treatment and facilitating communication between patients and their healthcare team.

Requirements
- Masters Degree in Social Work from a program accredited by the Council on Social Work Education.
- Licensed as Licensed Clinical Social Worker (LCSW) in the state of North Carolina.
- Three years of broad clinical experience in a healthcare setting for patients with complex medical conditions required.
- Ability to communicate effectively with patients, families, and support systems, promoting treatment plan goals.
