We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at Consulate Health Care. As an LPN, you will play a vital role in providing direct patient care to our residents and patients, assisting them in activities of daily living under the direction and supervision of a professional nurse.
Responsibilities:- Implement individualized treatment plans for each assigned resident
- Supervise Certified Nursing Assistants (CNAs) and participate in their evaluation
- Oversee Falls Program and Pressure Sore Program
- Provide nursing services, including conducting and documenting thorough evaluations of each resident's medical status
- Perform other duties as assigned
- Current, unencumbered, active state license to practice as an LPN
- Experience in Skilled Nursing/Rehabilitation facilities preferred
- Qualifications include being qualified, compassionate, and dedicated to a job well done
