Responsibilities:
- Providing compassionate nursing care to patients in post-acute facilities
- Monitoring patients' physical and mental conditions and tracking vital signs
- Performing resident intakes, documenting medical histories, and tracking health concerns
- Helping with medication administration and verification, tracking use and effects
- Cultivating a comfortable and safe facility for residents
- Tracking medical records and charts
- Assisting with IV setup, immunizations, and phlebotomy procedures
- Keeping track of medical supplies and maintaining inventory
- Being accountable for following and adhering to resident care plans
- Working with staff to orient and train new colleagues
Qualifications and Skills:
- Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN)
- Completion of approved LPN educational program
- High school graduation, GED, or equivalent
- At least 6 months of experience (LPN or similar role)
- Ability to lift a minimum of 50 lbs
