Key Responsibilities:
- Perform initial assessments in patients' homes, evaluating their health status, strengths, care requirements, and preferences to create personalized long-term care plans.
- Review medical records and health referral documentation.
- Input assessment data into electronic systems as required.
- Document any discrepancies or concerns identified during the assessment process.
- Facilitate continuity of care to minimize treatment gaps.
- Discuss available healthcare options, support services, and vendor choices with consumers.
- Coordinate communication between the Call Center and eligible individuals for health plan enrollment.
- Enhance collaboration with interdisciplinary care teams to optimize care coordination.
- Gather quality review data and necessary documentation for outcome assessments.
- Maintain an in-depth understanding of community resources and service networks.
- Meet established performance standards and criteria.
- Undertake additional responsibilities as directed by management.
Qualifications:
- Experience: Minimum of two years in a home care setting within the last decade, encompassing diverse experiences.
- Recent involvement in comprehensive patient assessments.
- Current Registered Nurse (RN) License in New York State.
Work Schedule:
- Full-time positions require a commitment of four days per week with designated start times.
- Part-time roles necessitate a minimum of five assessments weekly on flexible days.
- Comprehensive training is provided for a specified duration.
- Post-training availability for mentoring and shadowing is expected.
