Psychiatric Nurse Practitioner - Home-Based Evaluation Specialist
Job Summary:Customized Assistance Services (CAS) is seeking a skilled Psychiatric Nurse Practitioner to join our team as a Home-Based Evaluation Specialist. In this role, you will be responsible for conducting comprehensive psychiatric evaluations of clients in their homes, shelters, or other designated sites.
Key Responsibilities:- Obtain referrals and schedule evaluations for clients, coordinating with referring agencies and caseworkers.
- Review client records and perform comprehensive psychiatric evaluations, preparing detailed mental health reports and formulating service and treatment recommendations.
- Arrange for involuntary transport to the hospital of individuals who are a danger to themselves or others, in coordination with police, EMS, and hospital personnel.
- Refer cases to HRA's Office of Legal Affairs for guardianship, Guardian-Ad-Litem, and other legal interventions, executing Orders to Gain Access (OGA) as needed.
- Testify in court proceedings for guardianship and other legal interventions, as requested.
- Consult with Medical and Mental Health Directors on utilization of MHL Section 9.37 authority and attend office meetings, supervisory meetings, and training sessions.
- Comply with standards of care and HRA quality assurance standards, including timely submission of mental health reports and other clinical documentation.
- A valid New York State License and current registration to practice as a Registered Professional Nurse.
- Three years of full-time satisfactory experience as a Registered Nurse, with two years in a supervisory capacity within a hospital or other appropriate setting.
- For Assignment Level II, completion of a Psychiatric Nurse Practitioner program or equivalent, or current certification as a Psychiatric Nurse Practitioner by a national certifying body recognized by the New York State Education Department.
