As a key member of our staff, you will play a critical role in providing continuity of care for children through our family care management model. Your primary responsibilities will include developing, expanding, accessing, and linking resources in the community to meet the needs of the child throughout their experience in the system.
This position requires strong verbal and written communication skills, as well as the ability to document clearly and concisely. You must be able to assess, interpret, and provide care information for children and families, working effectively within a family care team managing children aged 0-17. A positive attitude, patience, respect, and cultural sensitivity are essential when dealing with challenging environments.
We offer a competitive salary range of $55,000 - $65,000 annually, depending on experience. Our comprehensive benefits package includes medical, dental, and vision insurance, as well as short-term disability and long-term disability coverage.
