About the Role:
We are seeking a skilled Advanced Practice Provider to join our team at AlignMed Partners. As a key member of our healthcare team, you will provide primary and urgent care services to nursing center residents in collaboration with other medical providers.
Responsibilities:
- Collaborate with the Director of Clinical Practice and/or the Area Clinical Practice Manager to achieve center-specific goals and priorities.
- Work collaboratively with Center Leadership to achieve AlignMed Partners' mission of improving the lives we touch through the delivery of high-quality healthcare and everyday compassion.
- Provide comprehensive care to nursing center residents, including primary and urgent care services.
Requirements:
- Graduate from an accredited School of Nursing program and licensed as a registered nurse.
- Certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing and a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP).
- A minimum of two years of experience working with adult or geriatric populations is preferred.

- Yearly average of sixteen thousand dollars per year in bonus through our no-cap Incentive Compensation Program (for providers working 40 hours per week).
