The Pharmacy Department at Dana-Farber Cancer Institute is committed to delivering exceptional patient care and advancing the field of oncology.

The Pharmacy Technician II, Medication Reconciliation is a key member of our team, responsible for providing critical support to pharmacy operations under the supervision of a licensed pharmacist.

This role involves conducting in-person or phone meetings with patients to update medication lists, ensuring accuracy and completeness of the medication list.

The successful candidate will also create and update lists of current medications, document changes in dose, frequency, or route, and flag medications for removal to prevent harmful drug interactions.

Effective communication with pharmacists and providers is essential to maintain patient safety.

The Pharmacy Technician II, Medication Reconciliation will also provide cross-coverage in the outpatient/specialty pharmacy as needed to maintain seamless patient care.

This role requires strong analytical and problem-solving skills, as well as the ability to work effectively in a collaborative team environment.

The ideal candidate will have a strong foundation in pharmacy technology systems and be able to troubleshoot issues independently.
