Position Overview
This role is accountable for establishing and nurturing ongoing relationships with Memorial Hermann Physician Network ("MHMD") physicians and their administrative teams concerning Memorial Hermann resources. The position collaborates with MHMD physicians to meet objectives outlined in established incentive programs and to gain recognition from national accrediting organizations (such as NCQA, Bridges to Excellence, etc.), insurance providers, or other relevant entities for achievements in disease management, population health management, medical cost control, and the adoption of Patient-Centered Medical Home (PCMH) principles. Responsibilities will include coordinating and overseeing application processes, monitoring adherence to established benchmarks, and providing recommendations for process redesign to enhance compliance and performance.Key Responsibilities
Minimum Qualifications
Education: Bachelor’s degree in a relevant field or four (4) years of related experience.
Licenses/Certifications:
A license in one of the following is preferred:
National Committee for Quality Assurance (NCQA)
Certified Professional in Healthcare Quality (CPHQ)
Certified Medical Office Management (CMOM)
Experience / Knowledge / Skills:
- Three (3) years of experience in healthcare with a solid understanding of physician requirements, practice patterns, and workflows.
- Strong communication and presentation skills are essential.
- Interpersonal skills are necessary to effectively engage and build robust relationships with physicians, staff, and all management levels.
- Excellent organizational and prioritization abilities; capacity to manage multiple projects simultaneously and achieve desired outcomes on schedule.
- Analytical skills are required to identify client needs and formulate action plans.
- Proficient in Microsoft Office applications - Excel, Word, and PowerPoint.
- Familiarity with major quality improvement initiatives (PQRI, NCQA, CI, HEDIS, etc.).
- Ability to travel to MHMD meetings and physician offices within the regional area.
- Acts as the primary liaison between MHMD and MHMD physicians and their office staff regarding the implementation and utilization of Memorial Hermann resources aimed at advancing PCMH and ACO initiatives.
- Responsible for the deployment of programs at the practice level, including but not limited to, introducing and educating practices on the value and use of reporting tools, patient registries, and the delivery of reports via our physician portal to enhance patient care.
- Facilitates regular improvement meetings to monitor, present, and discuss progress on action plans and milestone achievements.
- Maintains the necessary skills to analyze data and processes to identify and address undesirable patterns or trends in utilization or performance, securing resources to develop strategies for improvement.
- Collaborates with regional medical directors, management, and practices to identify organizational and structural challenges that impede the achievement of desired program outcomes.
- Documents action plans and details of visits and outcomes, reporting critical incidents and quality of care issues.
- Assists in resolving managed care reimbursement issues identified by providers and/or payers, including root cause analysis of fee schedules and contract reimbursement.
- Travels throughout the regional area to meet with MHMD physicians and office staff; attends office meetings and conducts presentations.
- Ensures that documentation produced and/or processed complies with state regulations and accrediting body requirements.
- Ensures safe care for patients, staff, and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary constraints, including time management, supply management, productivity, and quality of service.
- Promotes individual professional growth and development by fulfilling requirements for mandatory/continuing education and skills competency; supports department-based goals that contribute to the organization's success; serves as a mentor and resource to less experienced staff.
- Demonstrates a commitment to caring for every member of our community by creating compassionate and personalized experiences. Models Memorial Hermann's service standards by providing safe, caring, personalized, and efficient experiences to patients and colleagues.
