Cardiology Case Manager RN

We are seeking a skilled and compassionate RN Case Manager to join our Cardiology team at Kaiser Permanente. As a key member of our multidisciplinary team, you will work collaboratively with physicians and other healthcare professionals to provide high-quality care to our patients with complex cardiovascular conditions.
Key Responsibilities- Evaluate and identify patients' needs, developing individualized care plans to address their unique requirements.
- Collaborate with primary care physicians, specialists, and other disciplines to develop and implement comprehensive case management plans.
- Monitor and evaluate the effectiveness of care plans, making adjustments as necessary to ensure optimal patient outcomes.
- Coordinate interdisciplinary care, including utilization management, transfer coordination, and discharge planning, to ensure seamless transitions and continuity of care.
- Act as a clinical liaison with outside agencies, such as County CCS, non-plan facilities, and third-party administrators, to facilitate communication and coordination of care.
- Prepare reports, communicate program changes to staff, and develop protocols in accordance with state regulations.
- Advocate for patients and families, educating them on their conditions and empowering them to take an active role in their care.
- Develop and update training and educational materials, presenting to staff, members, and families to promote self-management and disease state education.
- Facilitate patients' return to normal daily activities by teaching and making referrals for outside services and continued care.
- Consult with internal and external physicians, healthcare providers, and discharge planners regarding continued care, treatment, or hospitalization.
- Participate in data collection and analysis of clinical outcomes and customer satisfaction standards, contributing to the formulation and implementation of action strategies and outcomes of care or customer service.
- Minimum two (2) years of clinical experience as an RN in an acute care or ambulatory care setting.
- Bachelor's degree or equivalent experience.
- Current California RN license and BLS certification.
- Demonstrated ability to utilize and apply principles, practices, techniques, and methods of utilization review/management, care coordination, transfer coordination, discharge planning, or case management.
- Working knowledge of regulatory requirements and accreditation standards.
- Computer literacy skills.
- Bachelor's degree in nursing or healthcare-related field.
- Case Management Certification or certification in the area of specialty.
- Heart Failure experience.
