Job Title: Spiritual Care Coordinator

Job Summary:

The Spiritual Care Coordinator will provide spiritual care to terminally ill patients and their families in the comfort of their own homes. This role involves assessing the patient's and family's spiritual needs and providing counseling to meet those needs. The Coordinator will also facilitate visits by local clergy, pastoral counselors, or other individuals who can support the patient's spiritual needs.

Responsibilities:

  • Assess hospice patients and support systems to identify spiritual needs.
  • Provide support for the spiritual needs of the patient/family.
  • Attend interdisciplinary team meetings and participate in the patient care planning process.
  • Meet mandatory continuing education requirements of the agency.
  • Promote Agency philosophy and administrative policies to ensure quality of care.

Requirements:

  • One (1) year experience as a Chaplain in a health care institution preferred.
  • Exceptional interpersonal skills.
  • Reliable transportation and current auto liability insurance, as well as a valid driver's license if a personal vehicle is used for work.
