Nurse Care Manager

LHC Group is seeking a skilled RN Care Manager to join our team in Tualatin, Oregon. As a key member of our home health care team, you will be responsible for providing high-quality patient care and leading a team of clinicians to ensure the best possible outcomes for our patients.
About the Role:- Provide clinical leadership and supervision to a team of RNs and other healthcare professionals.
- Develop and implement individualized patient plans of care, ensuring that patients receive the highest level of care and support.
- Coordinate and supervise the delivery of clinical services, including assessments, treatments, and education.
- Collaborate with physicians, families, and other healthcare professionals to ensure seamless care coordination.
- Ensure compliance with regulatory requirements, agency policies, and best practices in patient care.
- Receives referrals and ensures timely patient evaluations by assigning clinicians and plotting start of care visits.
- Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate, and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
- Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriate.
- Coordinates all aspects of care with all disciplines, physicians, durable medical equipment providers, caregivers/family members, transferring facilities, and any other applicable healthcare providers.
- Current RN licensure in the state of practice.
- Current CPR certification required.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
