Job Summary:

Create and maintain relationships with referral sources to generate new business and develop community standing. Coordinate referrals from all service lines and provide support to new and existing clients. Assist in coordinating staffing for client visits and admissions.

Responsibilities:
  • Work independently to triage calls and coordinate referrals from all Methodist Fremont Health service lines, and provide support and coordination of service to new clients as well as to existing clients.
  • Obtain and verify third-party policy information with the patient and/or referral source.
  • Assess the needs of clients referred to Home Health Care and Hospice determining appropriate service, department and discipline based on regulatory guidelines and funding.
  • Communicate and coordinate with management about a potential client's clinical condition, those requiring immediate attention, those with unique needs, and those with complex treatment plans.
  • Facilitate positive outcomes by providing education regarding client's role in working toward independence.
  • Triages and responds to calls concerning all Home Health Care and Hospice clients to include intermittent home care, maternal/child, mental health, health services, hospice, pharmacy and IV departments and service lines.
  • Fully engages referral sources by way of marketing and business development.
Requirements:
  • Graduation from an accredited School of Nursing required.
  • Current valid Registered Nurse (RN) License, valid compact multistate license, or a temporary permit while awaiting licensure required.
  • Minimum 3 years clinical experience required.
