Job Summary

VNS Health is seeking a skilled Nurse Field Supervisor to oversee the delivery of high-quality home health care services. As a key member of our clinical team, you will be responsible for providing case management support, developing care plans, and ensuring exceptional clinical outcomes.

Key Responsibilities
  • Provide clinical intervention and support to patients in their homes
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Conduct regular supervisory visits to patients' homes to assess their condition and provide training support
  • Communicate effectively with the clinical care team to ensure timely and accurate charting
  • Orient and supervise Home Health Aides (HHAs) in the field
  • Ensure compliance with regulatory requirements and VNS Health policies
Requirements
  • Current license to practice as a Registered Nurse in New York State
  • Minimum of two-year experience as an RN in a home care setting
  • Valid driver's license may be required
