Key Responsibilities- Implement the medical care plan established by the Registered Nurse (RN) or the individuals we support.
- Reinforce education on health-related subjects and medical procedures, such as diabetes management, medication administration, g-tube care, respiratory treatments, and oxygen use, initially taught by the RN.
- Monitor for any signs of health issues, document observations, and collaborate with the RN on subsequent care actions, while providing hands-on assistance with all activities of daily living (ADLs).
- Serve as an advocate for individuals during hospital stays and offer compassionate end-of-life care within residential settings.
- Contribute insights to the RN for the development and execution of personalized care plans tailored to the needs of each individual.
- Primarily work evening shifts, with some daytime shifts as required.
- Licensed as a Practical Nurse by the New York State Department of Education.
- Possession of a valid New York State Driver's License.
- Experience in long-term care and supporting individuals with intellectual and developmental disabilities is preferred.
- Ability to adapt to flexible scheduling, including evenings and weekends.
- Strong critical thinking skills and demonstrated leadership abilities.
