Key Responsibilities
  1. Implement the medical care plan established by the Registered Nurse (RN) or the individuals we support.
  2. Reinforce education on health-related subjects and medical procedures, such as diabetes management, medication administration, g-tube care, respiratory treatments, and oxygen use, initially taught by the RN.
  3. Monitor for any signs of health issues, document observations, and collaborate with the RN on subsequent care actions, while providing hands-on assistance with all activities of daily living (ADLs).
  4. Serve as an advocate for individuals during hospital stays and offer compassionate end-of-life care within residential settings.
  5. Contribute insights to the RN for the development and execution of personalized care plans tailored to the needs of each individual.
  6. Primarily work evening shifts, with some daytime shifts as required.
Qualifications
  • Licensed as a Practical Nurse by the New York State Department of Education.
  • Possession of a valid New York State Driver's License.
  • Experience in long-term care and supporting individuals with intellectual and developmental disabilities is preferred.
  • Ability to adapt to flexible scheduling, including evenings and weekends.
  • Strong critical thinking skills and demonstrated leadership abilities.
