Responsibilities

  • Provide intermittent skilled nursing services to patients in their homes
  • Direct, supervise, and instruct hospice aide staff in the provision of personal patient care
  • Assess and evaluate patient needs, identifying mutually agreed-upon goals for care plans
  • Report patient status, updates, and discharge planning to clinical leadership, attending physicians, and hospice physicians
  • Participate in Quality Assessment and Performance Improvement (QAPI) program through data collection and submission
  • Ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care

Requirements

  • Ability to work in a field setting and make sound nursing judgments
  • Experience with Microsoft Office
