Job Title: Community Health Outreach Worker-FIT

At TRIAD ADULT AND PEDIATRIC MEDICINE, INC., we are seeking a highly motivated and compassionate Community Health Outreach Worker-FIT to join our team. This role is a critical part of our Guilford County Formerly Incarcerated Transition Program, which aims to support individuals in successfully re-entering society after incarceration.

Key Responsibilities:
  • Connect residents with community resources that offer educational programming and other supportive services.
  • Understand and educate participants on medication access options and services.
  • Assist in provider-patient care relationship building.
  • Help participants understand how to use resources to find health coverage or apply for assistance when uninsured.
  • Service locations for this position include clinics, home visits, transition centers, or other community settings.
  • Participate in the Transition Care clinic network conferences, trainings, and meetings.
  • Participate in NC FIT Program phone calls, webinars, and meetings.
  • Participate and maintain required trainings to engage the population.
  • Represent the organization at local community meetings, including but not limited to: Guilford County Re-entry Council, NC Works Partners Meeting, Guilford Community Care Network, Housing coalitions, Partners Ending Homelessness, etc.
Requirements & Experience:

We are looking for an individual with good written and oral communication skills, the ability to work well independently and with a team, and basic computer/word processing skills. Previous experience in peer support work is desired. Some early morning, evening, and weekend work may be required. Demonstrated possession of knowledge, skills, and abilities as stated above through at least one year of experience in performing similar tasks in a hospital, clinic, nursing home, medical corps, prison, or other social or rehabilitation services.

Special Requirements:

  • History of incarceration and successful re-entry into the community after at least three years from release.
  • Requires a valid North Carolina driver's license with a three-year acceptable driving record.

Education:

High school diploma or GED equivalent preferred. Previous experience in peer support work desired.
