VNS Health is seeking a highly skilled and experienced Clinical Supervisor to lead our Hospice Intake team. As a key member of our clinical leadership team, you will be responsible for overseeing the day-to-day operations of our Hospice Liaison Intake process, ensuring that our patients receive the highest level of care and support.
Key Responsibilities:- Supervise and coordinate the work of our Hospice Liaison team, ensuring that all aspects of the intake process are completed efficiently and effectively.
- Develop and implement quality improvement initiatives to enhance patient care and outcomes.
- Collaborate with our Quality Improvement and Education teams to ensure that our clinical practices are aligned with best practices and regulatory requirements.
- Communicate effectively with our Care Management and Hospice agencies to ensure seamless coordination of care.
- Recruit, train, and evaluate staff to ensure that our team is equipped to provide exceptional patient care.
- Minimum of three years of progressive clinical experience in home care, Hospice, or a related field.
- Demonstrated knowledge of regulatory and reimbursement requirements governing Hospice.
- Effective oral and written communication and interpersonal skills.
- Prior experience managing multiple teams and staff located in multiple locations.
